I (we), upon joining the Rose and Max Rady Jewish Community Centre (Rady JCC) agree to abide by the policies, rules, and regulations of the organization. I (we) understand that membership services may be limited or terminated upon violation of these rules. I (we) assume all risks and hazards incidental to the conduct of the activities, including use of facilities and equipment and transportation to and from these activities. I (we) understand and acknowledge the Rady JCC is not liable for any loss or theft of personal property wheresoever located in, but not limited to personal property left in lockers, storage, cloak room, family change area, parking lot or in any other area of the Rady JCC and the Asper Jewish Community Campus.
I (we) do hereby release and hold harmless, the Rady JCC and/or organizers, sponsors and anyone connected with the program or activity. I understand and acknowledge that I (we) are bound by all the terms and conditions related to membership and use of the facilities as set out in the Membership Handbook that I (we) hereby acknowledge receiving.
I (we) permit the free use of my name and family members' name and photos listed on this application in broadcasts, newspapers, brochures and any form of communication to which such use may be applied.
I (we) consent to use of my and my family's personal information for the purposes as outlined in the privacy policy (
www.radyjcc.com/privacy).
I (we) understand that membership is for 12 months from the enrollment date (except as noted below * for those choosing a monthly payment plan).
Membership is neither refundable nor transferable.
We trust you have applied for the membership category that is appropriate for your age and maritial status. Membership is subject to revocation without refund if you are not qualified for the membership category for which you have applied.
* Members choosing to pay on a monthly payment plan. Membership will be paid in monthly installments. At the end of the 12-month period, the membership will be automatically renewed on a month-to-month basis. In the event I (we) wish to resign my (our) membership after the initial 12 months, I (we) will inform the Membership Department in writing at least 30 days in advance of the discontinuation of my (our) membership. I (we) acknowledge a failure to provide the required thirty (30) days notice will result in my (our) being liable for an additional thirty (30) days of membership fees.
